New patient information

Are you new to acupuncture? Don't worry! We are here to inform you and make you feel as comfortable as possible. As with most health practitioners, the first visit to an acupuncturist usually begins with the practitioner taking a detailed history.

Traditional Chinese medicine takes a more holistic approach to patient care than Western medicine. In this context, you may be asked questions about your sleep habits, your ability to tolerate heat or cold, or about your dietary habits that appear unimportant, but are actually vital to the type of care you will receive.

After reviewing your history, the practitioner will begin diagnosing your ailment. Depending on your condition, you may be subjected to an examination of the tongue, as well as an examination of the pulse - a major diagnostic technique in traditional Chinese medicine.

Using all the information obtained during the history and diagnosis, the practitioner will then determine the cause of your symptoms. Depending on the condition, needles will be inserted into specific acupuncture points on the body. The acupuncturist may use electrical stimulation to enhance acupuncture's therapeutic effect.

Depending on the seriousness and the length of your condition, your first visit may take between one and a half to two hours. It may take several visits to see significant improvement or cure your condition. As with any treatment plan, however, make sure that your questions are answered completely, and that the treatment plan seems viable and reasonable. If you don't understand a particular technique or type of treatment, make sure to ask.

You can find below the links to forms that you can download, print and fill out. The filled out forms can then be either faxed or mailed to the clinic for an appointment.

The forms are set up as Portable Document Format (pdf)* files. You will need the FREE Acrobat Reader browser extension or application from Adobe to view and print these forms. See below for a link to download Acrobat if your browser does not already include this extension.

Note:  When you click on the links below, they will open in a new window to load the form/or launch the Adobe Acrobat Reader application. The window will remain open behind the new one. In case the window does not open and if you are asked to save the file, then you do not have the Adobe Acrobat Reader plug-in installed for your browser.  Installing the plug-in (see below) will allow you to view and print all PDF files directly from your browser window.

If you have any trouble downloading and printing our forms, don't worry. Forms are also available at our office. You can contact us by phone or email us if you would want the forms mailed to you, or you can pick them up, fill them out at our office before your first visit.
